Medical Plan
Fulltime employees have the opportunity to elect Point-of-Service (POS) plan medical coverage. Employees will be responsible for a contribution to participate in the medical coverage. The amount will be automatically deducted from the employee’s pay on a pre-tax basis. The Point-of-Service (POS) plan is a managed care medical plan that requires the employee to choose a Primary Care Physician (PCP) who will coordinate all of your medical care. The Point-of-Service (POS) plan gives the employee the opportunity to choose either in-network care or out-of-network care.
Dental Plan
Fulltime and part-time employees may elect dental coverage. Employees are responsible for a pre-tax contribution to participate.
Vision Plan
Fulltime and part-time employees may elect vision coverage. Employees are responsible for a pre-tax contribution to participate.
Group term life insurance
Fulltime employees receive hospital paid group term life insurance. They have the opportunity to elect to purchase additional coverage for themselves and spouse and/or children at this time.
Accidental death and dismemberment insurance
Included with life insurance coverage, employees receive accidental death and dismemberment coverage.
Short and long term disability
Short and long-term disability insurance is available to fulltime employees. There is no cost to the employee for this coverage.
Paid Days Off
Paid Days Off (PDO) are provided for scheduled time off such as vacations, unscheduled instances of illness, personal emergencies and personal holiday.
Holidays
Fulltime employees are eligible for seven paid holidays in a calendar year: New Year’s Day, Memorial Day, July 4th, Labor Day, Thanksgiving Day, Christmas Day and a Personal Day.
Pension Plan
Currently, certain eligible employees participate in Kane Community Hospital’s Defined Benefit Pension Plan. The Pension Plan is based on years of service and annual salary.
403(b) Plan
Currently, eligible employees may elect to contribute to Kane Community Hospital's 403(b) Plan.
Funeral Leave
Employees may be eligible for funeral leave. Funeral leave is granted depending on an employee’s relationship to the deceased.
Employee Assistance Program (EAP)
Kane Community Hospital offers a program that provides counseling and guidance to individuals who may need confidential assistance with personal problems.
Direct Deposit
Employees are paid biweekly using the direct deposit method for paying compensation.
This summary of benefits applies to all regular fulltime employees. This summary of benefits does not apply to staff physicians. This summary may help employees choose a combination of benefits that is best for them and their family. Employees may enroll eligible dependents for certain coverage. Costs are shared between the employee and Kane Community Hospital. Specific levels of some benefits may be provided at no cost to the employee. The value of some benefits varies depending upon income and other factors.
